Every client is to be treated the same regardless of your opinion of the severity of their issue. Never assume a client is able to take minimal instructions on the advice you are giving, if they knew they would not come to you. By being polite and empathetic, you can turn down a client without receiving a rude response from them. Always smile, it is the most universal way of greeting someone without offending people of different nationalities.
